The Coastside Chamber is collecting information, anonymously, to aid our local governments with data to help them apply for future funding related to storm and power events. This survey may be filled out by any Coastside resident or business owner. Please only fill out the form once. If both your home and business sustained damage, you can include both on the form, as there is a section dedicated to each.
